If you are newly diagnosed with cancer, and there is an effective treatment out there, you can either take it or go into a trial. The choice is entirely yours. It's also important to note if a patient can withdraw from a trial at any time and choose another treatment.

Doctors want to be able to design trials to look at multiple potential treatment combinations and use biomarkers to determine who is responding to what medication. At the end of the day, when they enroll the last patient in the study they want to be able to treat that patient optimally depending on the patient's disease characteristics. Using a Bayesian approach, the trial design exploits the results as the trial is ongoing and adapts based on these interim results. That kind of thing is an anathema in the standard approach.

Ascorbic acid (vitamin C, ascorbate) has a controversial history in cancer treatment, ... Observational reports described ascorbate, given in pharmacologic doses of 10 g daily, as effective in treating some cancers and in improving patient well-being. Subsequently, the same dose had no effect on patient well-being and survival in two double-blind placebo-controlled trials, and ascorbate was discarded as a treatment modality.

Unfortunately, less than five percent of people diagnosed with cancer will be treated through enrollment in a clinical trial. Our organization is very supportive of the Coalition and its efforts to accelerate research. We believe that in recent years patients have received better treatment options in trials, and if we can educate people about clinical trials I am confident that we can increase enrollment in these high-priority colon cancer trials.

For depression, there is no single medication or therapy that has been found to work as a primary treatment for most patients. We found that people with depression who have increased activity in one area of the brain and decreased activity in another in response to emotional stimuli are more likely to respond to a specific treatment--cognitive therapy. If this finding holds true, we may be able to predict what therapies will be most effective for individual patients by using imaging technology, bypassing the lengthy trial and error process that is often necessary to find the right treatment.

Under the current medicare system of our country, it is not rare for a person who catches a major disease like cancer to spend his or her lifetime savings on treatment. Many are forced to choose giving up treatment, just waiting for death.

Targeted cancer therapies, therapies that exploit the particular weaknesses of different types of cancer, show enormous promise for the next generation of cancer treatment. It's not practical to wait weeks after breast cancer surgery, for example, to decide how aggressive follow-up treatment should be. The effectiveness of many therapies depends on timing.
