High-deductible health plans, with or without health savings accounts, are being designed to try and control health care costs from the demand side, and they are really quite controversial at this point.

Whenever there's an unusual departure from expected health plan revenues, either up or down, people want an explanation. And there's no shortage of possible reasons for the income increase. Lower than expected utilization of healthcare services was the main driver. Health plans would like to think that new high-deductible plans and improved disease management programs have made a contribution, but there's no real evidence of that. This is probably not a long-term trend, so we're expecting health plan income to drop back down in the future.

Many people are confused by what it means to be high-risk. It doesn't have anything to do with being at higher risk for exposure to the flu virus, such as teachers or bus drivers. People who are at high-risk are those whose health could be severely impacted if they were to contract the flu, such as those who have pre-existing health problems or a compromised immune system.

A lot of people who get laid off from corporate jobs end up doing some freelance work, ... But people who are coming from a job with a paycheck are not knowledgeable about the expenses they can file on Schedule C. They should familiarize themselves with those deductions. If they have considerable self-employment income, they can set up a Keogh or SEP IRA to shelter some of the income from taxes.

We're encouraging people to hang in there. In Missouri, the flu season doesn't peak until January or February, and it takes about two weeks for the vaccine to take full effect, so there really is plenty of time for most people. There still is some concern for high-risk groups, who should have been first in line. Those are the folks we're encouraging to stay on the line with physicians and with county health departments to find out where the vaccine will be. But for the rest of us who are healthy and don't have a chronic illness or health problems, there really is plenty of time. We just need to be patient.

A successful person isn't necessarily better than her less successful peers at solving problems; her pattern-recognition facilities have just learned what problems are worth solving.

People covered by Medicare tend to have more health problems than younger people. But many of them can ill afford the medications they need. The result is that they may not take them, allowing readily controllable health problems to evolve into unmanaged crises.

Being overweight and obesity are major risk factors for many chronic diseases for South Dakotans of all ages. When people are overweight or obese, they have more health problems and more serious health problems, in addition to higher health care costs.

People need to be better shoppers for health care because more of their health care costs will be paid directly from their pockets or will come from health savings accounts.

Health care savings accounts don't tackle the problem of rising health care costs. This is not an attempt to rein in the cost of health care. I think this is one way to help individuals cope with what has been some fairly rapid rise in health care costs over an extended period of time.

Most of the people we see don't want to live in a shelter and feel safe in their own little camp. Experience has taught me that almost 100 percent of these people suffered abuse as children. Well over half have emotional, mental problems. Most have drug and alcohol problems.

Gay people have to report domestic partner health insurance as income to the IRS, but married spouses don't have to report their health coverage as income. Not only do we earn less, we are less able to keep what we earn.
