Cases we'll be looking at concern ingestion of Vioxx for 18 months or more by patients with limited heart risk factors.

The jury's decision confirms that there is no medical or scientific evidence showing short-term use of VIOXX increases the risk of heart attack and contributed in any way to the unfortunate death of Richard Irvin. Mr. Irvin only took VIOXX for less than a month. He suffered multiple long-standing risk factors for a heart attack including partially clogged arteries. Mr. Irvin would have suffered a heart attack when he did, whether he was taking VIOXX or not.

For a relatively small set of cases that involve patients who used Vioxx for over 18 months, we will take a close look.

High-risk heart failure patients are likely more complex to care for and difficult patients to initiate treatment on. I don't think physicians are giving up on their patients. But they may be struggling with some complex cases.

Patients with multiple risk factors, including diabetes, face a greater threat of heart attack and stroke, so reducing their risk is critical.

The clinical study Merck conducted that lead it to voluntarily withdraw the drug only revealed an increased risk after 18 months of daily use. Mr. Humeston only took Vioxx intermittently for less than two months. The science just isn't there to support his claim.

And we were really surprised to find out in our research last year that 50% of the women that attended our event had abnormal levels of risk factors and didn't know it. We found surprising consistency across the studies. In particular, we found that well known risk factors for heart disease also are risk factors for cognitive decline, and that physical activity may reduce risk for cognitive decline and dementia in older adults.

The results of these tests would allow practitioners to counsel patients on stroke risk and to warn patients that specific environmental factors, such as oral contraceptive use, diet or smoking, may be particularly harmful.

Even in long-term use cases, the plaintiffs are going to have to show that it was this drug and not their pre-existing risk factors and medical conditions that caused their heart attacks. That's just going to be a steep hill for these plaintiffs to climb.

Not all patients become depressed after having a heart attack. Depression should not be considered a normal reaction after a heart attack. Clinicians and patients should be aware that depression is an important risk factor for adverse outcomes after a heart attack.
