After spending three years of my life looking into this, I am more convinced than ever that the U.S. government's responsibility for the drug problems in South Central Los Angeles and other inner cities is greater than I ever wrote in the newspaper.

Dense central cities have some advantages, but they also have some tremendous disadvantages: congestion, pollution, crime, all the things that central cities have been criticized for in the past 100 years. The same is true of sprawl -- it has all kinds of problems, but I don't know that they're any worse than any other part of the urban area.

When large numbers of poor families started showing up in the central valley of California, and also in Los Angeles, at a time when the federal government was providing very little assistance, then the burden of caring for those new residents fell to those towns and cities, and sympathy dried up pretty quickly.

Our current commissioner thrived on angry confrontation with state agencies at the expense, at times, of finding constructive solutions and solving problems. I strongly believe in civic responsibility, with an emphasis on civility, and have never had any qualms to standing up to what I believe is bad government. The prescription drug plans were able to negotiate discounts and rebates that came in larger than we thought, and this has helped mitigate what drug spending would have been. It doesn't mean drug spending won't continue to grow, but it has helped to temper that growth.
