There is something not quite right about letting a patient dictate whether or not they take medicine. Are mental patients the people to whom we should be leaving the decisions?

Much of the increase in generic drug use advocated by Express Scripts involves switching patients to medicines different from those prescribed by their physicians. The physicians of one class feel the patients and go away, merely prescribing medicine. As they leave the room they simply ask the patient to take the medicine. They are the poorest class of physicians.

Often, patients show mental illness and insobriety at the same time. People say that we should hold off on treatment until a patient is clean and sober, but it doesn't work that way.

It's no secret that there is an epidemic of chronic illness in this country, and more and more Americans are turning to complementary medicine for relief. Patients deserve a therapeutic environment they can trust. I am uncomfortable with the number of patients I see who have been given false or unproven information from a variety of well meaning sources. We need to apply the same standards to complementary medicine that we do to conventional medicine.

What we're trying to do is to see whether telehealth is an appropriate way to follow patients with dementia, ... The thing about dementia is once you've seen the patient and examined the patient neurologically, most of the followup is examining mental status, which we can do over telehealth. Talking to the patient and the family on how things are going and making some changes or suggestions, be it modifications in the environment or medication treatment for some of the behaviours?that works well over telehealth.

We've got sick babies, sick old people and everything in between. We've seen strokes, chest pain, diabetes patients passing out, seizures, people without medicine, people with the wrong medicine. It's been busy.

Jake has done a good job for us. He's able to spot his pitches and dictate to the hitters instead of letting them dictate to him; he's pretty consistent.

Patients need to know that depression fluctuates over time and can be treated effectively with medicine and/or psychotherapy. If they can be persuaded to give it time, they may find that they choose differently when making the decision clear of depression symptoms. That's all we want: for doctors and patients to make the best decisions on a case-by-case basis.

The change in medicine over the last 50 years, since our last hospital was built, has largely been change from predominantly in-patient medicine to an emphasis on out-patient.

These first two weeks the system has just been overwhelmed. On January 2 I spent the day helping a mental health patient get her medicine. It required prior authorization and it took me three hours to get that authorization.

This is a very significant threat to patients' rights in the United States. We need to protect the patient's right to use their own religious or ethical values to make medical decisions.
