What we're trying to do is to see whether telehealth is an appropriate way to follow patients with dementia, ... The thing about dementia is once you've seen the patient and examined the patient neurologically, most of the followup is examining mental status, which we can do over telehealth. We have standard staff-to-patient ratios and as the need increases. If the volume doesn't change, why would we increase the staff?

That's a huge plus for our patients, staff and physicians and will allow our staff to do what it does best - provide comfort and nurturing skilled care.

County, state and federal funding is down, patient needs have reached an all time high as the nation's poorest are unable to obtain health care insurance, and emergency medicine physicians are struggling to keep up with patient demand, especially in the absence of compensation.

In theory, in the private sector, with the L-code system, I know the end dollar amount that I can be reimbursed for a patient, so the less I see that patient in terms of clinical hours, the more profit is generated. That's not good in a rehab world.

The change in medicine over the last 50 years, since our last hospital was built, has largely been change from predominantly in-patient medicine to an emphasis on out-patient.

What bothered me is that I just got a memo from the (American) League stating that when (umpires) meet it has to be conclusive. When an umpire in the middle of the field overrules it, that word conclusive (comes to mind). That's a difficult call for me to swallow right there. That was kind of my point.

The medical staff were clear in their message that the status quo was not adequate. People are proud of how (the hospital) got here and what it means. We've got more technology involved in patient care now. There will come a time when we'll have to renovate it or replace it.

I'm glad that she's been patient enough and not kind of succumb to what a lot of coaches succumb to. Which is at the first sign of success they jump up to what they think is a greener pasture -- a big-time job somewhere else. But she's been patient enough to kind of build something that's going to be long lasting.
