The alarming thing in China is the almost total absence of primary care. Even in cities, there are no independent doctors' offices or neighborhood clinics, so people have to go to the hospital for every health care need.

Occidental is a real community clinic and a lot of people who come here come from not only the town but from surrounding cities. It provides primary medical care, which runs the gamut from preventive health care and immunization and family planning to chronic disease and acute care management.

The President spent surprisingly little time talking about health care in his address, an issue of primary concern to everyone. But the ideas he did espouse are more about saving money than getting people health care.

Where alternatives are available, this raises questions about whether a portion of these types of patients can be appropriately treated in non-emergency department settings, such as primary health care clinics.

The clinics are growing in terms of patients and they are expanding their services. Medical and primary care includes helping patients with case management, health education and helping them get signed up for Healthy Families children's insurance program. It's federally funded and was started several years ago. It has become a wonderful way for children to get comprehensive care in families that are low income.

There's a lack of health care services for that population. Emergency rooms and the (health) department are often their primary source of care.

Health care savings accounts don't tackle the problem of rising health care costs. This is not an attempt to rein in the cost of health care. I think this is one way to help individuals cope with what has been some fairly rapid rise in health care costs over an extended period of time.
